An expensive condition

Even for those of us with Cadillac health care plans, having chronic illnesses is expensive. And if obesity leads to the development of several chronic health conditions including type 2 diabetes, obstructive sleep apnea, hypertension, hypercholesterolemia and chronic arthritis, you could easily spend thousands of dollars yearly on medications and devices.

If you have type 2 diabetes, you know the pharmaceutical companies are spending millions of dollars trying to sell you very expensive medications. And if you require insulin, you very quickly become aware of the fact that insulins are not available in generic form, and a vial of insulin will costs $300. Because type 2 diabetes is characterized by insulin resistance, the chances are that when insulin is used, the necessary doses will be very high, requiring several vial of insulin monthly.

Results

For those patients on insulin or multiple pills for diabetes, the changes we make in your diet will yield benefits in reducing medications within days to weeks. Many of our patients have commented that they saved so much on drug costs that it more than payed for the program within the first three months.

For those individuals who are not yet on medications, the improvements in health brought about by sustained weight loss will prevent the need to start on medications.

Cost

The health transition clinic program is labor intensive.  Substantial and dramatic changes in lifestyle will often lead to marked changes in need for prescription medications.  By pairing lifestyle change (typically non-covered services) with close medical supervision, covered by health insurance, many people achieve better health with less medication.  The program is divided into 3 phases: A four month induction phase, followed by a six month augmentation phase.  Finally, there is an indefinite maintenance (graduate program) phase.

Phase 1

The first phase of the program is the four month induction phase.  During this phase patients initiate a very intense low carbohydrate diet, with very close monitoring of blood sugars, blood pressures, and medications.  There are monthly group visits, and weekly nurse visits.  The frequent interactions allow adequate monitoring, and often frequent medication changes, while providing accountability necessary to be successful.  There is a subscription fee of $220 per month for four months.  In addition to the subscription fee, typical physician evaluation and management fees (E&M codes) are charged to your insurance at each of the 4 sessions to cover the physician (this is covered by all health insurance, copays and deductibles apply).  Additionally, nurse visits are charged to health insurance, copays and deductibles again apply.

Phase 2

The second phase of the program is the 5-6 month augmentation phase.  During this phase, we attempt to address all of the issues likely to interfere with your progress.  We focus significant attention on building a sufficient library of recipes to allow you to enjoy this lifestyle, without feeling deprived or bored.  We continue to provide enough one on one attention to maintain a high degree of accountability.  We introduce and focus attention on all the behaviors necessary to have long term success, introduce exercise,  therapeutic fasting, strategies toward eating out at restaurants as well as family functions and traveling. There is a subscription fee of $80 per month for five months.  In addition to the subscription fee, typical physician evaluation and management fees (E&M codes) are charged to your insurance at each of the 4 sessions to cover the physician (this is covered by all health insurance, copays and deductibles apply).  Additionally, nurse visits are charged to health insurance, copays and deductibles again apply.

Phase 3

The third phase of the program is the indefinite maintenance phase.  We have previously called this the graduate program.  This open-ended program is offered to anyone who has completed the induction and augmentation phases.  Its focus is to maintain accountability while re-amplifying the messages heard throughout the program.  There is a subscription fee of $10 per month. In addition to the subscription fee, typical physician evaluation and management fees (E&M codes) are charged to your insurance at each of the 2 yearly sessions to cover the physician (this is covered by all health insurance, copays and deductibles apply).  Additionally, monthly nurse visits are charged to health insurance, copays and deductibles again apply.
